We will gladly answer any questions you may have about Camp Hope. We do have a phone
with an answering machine where you can leave us a message with your name, mailing
address, or email address to get back to you. However, our best connections are made when

Registration forms are pretty extensive. We need all of this information to properly care
for each child, so please, please fill out all of the forms completely and return them to us no
later than June 1. Sometimes the physicians' form must be sent in later to accomodate
appointments, but they should be sent as soon as possible. We supply all of your child's
diabetes care needs at camp, so needless to say, we need to know what to order well in
advance of the first day of camp. Registration and Medical Forms may be obtained by
